New Patients

Joining Clearview Is Four Simple Steps.

Switching to a family doctor who actually knows you shouldn't be a paperwork ordeal. Here's exactly how becoming a Clearview patient works — start to first visit.

The Process

From First Request to First Visit.

No portal to wrestle with, no long forms before you’ve even spoken to us. One request, one call, and we handle the rest.

1

Send a Request

Use Request an Appointment and choose "New-Patient Appointment." It takes about a minute — no account, no portal login to set up first.

2

We Call You Back

Our front desk calls within one business day to find a time, answer questions, and confirm which provider is the right fit for you or your family.

3

Records & Insurance

With your okay, we request your records from your previous provider and verify your insurance ahead of time, so nothing is a surprise at check-in.

4

Your First Visit

We book new patients a longer first appointment so your provider can review your history, hear your goals, and actually get to know you.

Before Your First Visit

What to Bring.

A little prep makes your first appointment smoother — and lets your provider spend the visit on you instead of on paperwork.

Insurance questions? Call (970) 555-0148 with your member ID and we’ll confirm we’re in-network before you book. Clearview accepts most major commercial plans plus Medicare and Medicaid.

Insurance & Payment

Straight answers on coverage.

Clearview is in-network with most major commercial insurance plans, Medicare, and Medicaid. Because plans and networks change, the surest way to know your specific coverage is to call the front desk at (970) 555-0148 with your member ID — we’ll check before your visit so there are no billing surprises.

Don’t have insurance right now? Ask us about self-pay pricing for office visits and common labs. We’d rather see you and talk it through than have cost be the reason a small problem waits until it’s a big one.

Frequently Asked Questions

Are you accepting new patients?

Yes — Clearview is accepting new patients of all ages, from newborns to seniors. Use the Request an Appointment button and choose "New-Patient Appointment," and our front desk will call within one business day to get you scheduled and help transfer your records from a previous provider.

Can I be seen the same day if I'm sick?

We hold same-day appointments open every weekday for sick visits — colds, flu, infections, rashes, and minor injuries. Call as early as you can, or send a request, and we'll get you in with a Clearview provider rather than sending you to urgent care.

What insurance do you accept?

Clearview accepts most major commercial insurance plans as well as Medicare and Medicaid. If you're not sure whether we're in-network for your plan, call the front desk with your member ID and we'll check before your visit so there are no surprises.

Do you see both children and adults?

We do — that's the point of a family practice. The same team that handles your child's well-child visits and school physicals also manages your annual physical and your parents' Medicare wellness visits, so your whole family's care lives in one place.

Do you offer telehealth visits?

Yes. Established patients can book secure video visits for follow-ups, medication check-ins, and simple concerns that don't need an in-person exam. Same-day sick visits and anything requiring labs or a physical exam are seen in the office.

Ready to make Clearview your family's practice?

Send a new-patient request and our front desk will take it from there — records, insurance, and a longer first visit all handled for you.

Takes about a minute. Our front desk confirms your time within one business day — nothing is charged to request a visit.
